dizziness that remains non specific despite thorough history, examination, evaluation is known as:
persistent postural perceptual dizziness (PPPD)
persistent postural perceptual dizziness (PPPD) symptoms:
non vertiginous dizziness or imbalance that worsens with personal motion, upright positioning, and movement of objections, upright positoning
movement of objects in surrounding enviornment.
symptoms must be present most days and for at least 3 months
persistent postural perceptual dizziness (PPPD) is often
preceded by other vestibular process (BPPV, vestibular neuronitis, vestibular migraine, stroke
trauma (concussion, traumatic brain injury)
infection
certain psychiatric conditions (anxiety, panic disorder, major depression)
treatment of persistent postural perceptual dizziness (PPPD) is with
–SSRI or SNRI
75% of pts have anxiety or depressive symptoms.
–also treat with balance training core stabilization and desensitization exercises - PT/OT.
how long to treat persistent postural perceptual dizziness (PPPD)
SSRI and SNRI takes 8-12 weeks to produce a clinical response and if effective treat for 1 year .
positive response to medications does not depend upon the presence of psychiatric medications.
