Features
A
Can be:
- localised, e.g. diverticulitis, appendicitis or
- generalised due to intraabdominal sepsis following perforation of a viscus.
Can occur spontaneously with ascites.
Typical signs are as for perforated peptic ulcer.

peritoneal fluid culture and CT scan

Management
A
Surgical intervention usually required
Antibiotics—cephalosporin or amoxy/ampicillin + gentamicin + metronidazole (IV)
