Where is peripheral IV generally located?
Located on arms, at elbow or hand, in small veins
How often are peripheral IVs changed?
Every 4 days to prevent infection
What does intermittent use of a peripheral IV require?
A saline lock
What is a saline lock?
Short section of tubing at end of IVC, keeps saline inside, is not removed
What is required for a saline lock?
- q8h saline flush when IV not in use, and saline flush before/after meds
- clear dressing covering lock (to assess site)
- marked with insertion date, gauge, initials of RN
2 Features of continual use peripheral IV?
- fluids are given over an extended period of time so no flushes needed
- an infusion pump is generally needed to reduce errors
Describe the IV complication Phlebitis?
Inflammation of the vein, graded 1-4 (4 being the worst)
Treatment for IV phlebitis?
- discontinue IV
- Assess and document
- warm pack
Symptoms of IV phlebitis?
Warmth and redness
Describe the IV complication Infiltration?
IV has exited the vein and entered interstitial fluid
Symptoms of IV Infiltration?
swelling and no blood return
Treatment for IV Infiltration?
- discontinue IV
- elevate extremity
Describe the IV complication Obstruction
A small blood clot which causes obstruction of fluid
Symptoms of IV Obstruction?
fluid isnt running correctly
Treatment for IV obstruction?
- administer low dose Ateplase to dissolve thrombus
- flush IV
