What does the acute/intermediate phase of burn care include?
Care from beginning of diuresis to wound closure
What are cardiopulmonary/fluid/electrolyte priorities during acute/intermediate burn care?
- monitor vitals
- monitor Na and K
- daily weight
What are immunity and infection priorities during acute/intermediate burn care?
- monitor drainage, odor, erythema, swelling
- administer tetanus shot
- antibiotics PRN
What are GI priorities during acute/intermediate burn care?
- administer TPN or enteral feeding with THREE TIMES normal protein and TWO TIMES calories
- schedule painful procedures away from meals
What are pain priorities during acute/intermediate burn care?
- opioids 30 min before wound care
- control anxiety
What are kidney injury priorities during acute/intermediate burn care?
- monitor urine output
- monitor globular filtration rate
What are muscoskeletal priorities during acute/intermediate burn care?
- monitor for compartment syndrome through: pallor, sensation, movement, cap refill, pulses
What happens to fluids during acute/intermediate phase of burn?
- fluid re-enters vascular space from interstitial space
What does the fluid shift during intermediate/acute phase of burns cause?
hemodilution and increased urine output
What happens to electrolytes during acute/intermediate burn phase?
as urine output increases, sodium is lost (hyponatremia), potassium shifts into cells (hypokalemia)
How often must patients response to fluid therapy be monitored (acute phase burn)?
hourly - via heart rate, urine output and blood pressure
