What is PAD?
Atherosclerotic disease of the lower extremities
What is the main presentation?
intermittent claudication
What are the 6 Ps of an acute arterial embolism from PAD?
pain, paresthesias, paralysis, pallor, pulselessness, poikilothermia
What dos rest pain indicate?
advanced disease
Where will claudication be if it’s the aortic bifurcation/common illiac artery?
Buttock, hip, groin
Where will claudication be if it’s the femoral artery (or it’s branches)?
thigh, upper calf
Where will claudication be if it’s the popliteal artery?
lower calf, ankle, foot
Where will claudication be if it’s the tibial and perineal arteries?
foot
What will the skin be like on PE?
cool to touch, thin, shiny
What will pulses be?
decreased or absent
How is PAD diagnosed?
ABI
What is the ABI in PAD?
less than 0.9
If the ABI is > 1.2 what does that indicate?
calcified vessels
What is the gold standard diagnostic test?
arteriography, clinically only done if revascularization is planned
What are the 2 other tests to diagnose?
Duplex U/S and hand held doppler
