What is the main cause of PUs?
H.pylori
What are the risk factors for PUs?
H.pylori, NSAIDs, smoking, stress, alcohol
How would you diagnose a PU?
Upper GI endoscopy
What are some differential diagnoses for DUs?
Non-ulcer dyspepsia, duodenal Crohn’s, TB, lymphoma, pancreatic cancer
What are some potential complications of PUs?
Bleeding, perforation, malignancy, decreased gastric outflow
How do you test for H.pylori?
Urease breath test
What lifestyle modifications would you advise?
Weight loss, smoking cessation
How would you manage a peptic ulcer?
Stop NSAIDs and/or eradicate H.pylori
What drugs eradicate H.pylori?
Clarithromycin/metronidazole + amoxicillin + omeprazole
How do gastric ulcers present?
Epigastric pain that’s worse on eating and relieved by antacids
How do duodenal ulcers present?
Epigastric pain after meals and at night that’s relieved by eating or milk
What drugs can help with peptic ulcers?
NSAIDs, SSRI, Corticosteroids, Bisphosphonates
What else can cause peptic ulcers?
Increased acid production, NSAIDs, Mucosal ischaemia
