penicillin Flashcards
types of penicillins
- penicillins
- broad spectrum
- penicillinase resistant
- extended spectrum
- beta-lactamase inhibitors
indications for basic penicillins
- staph infections
- severe wound & respiratory infections
bacteriostatic
inhibit growth of bacteria
bactericidal
kill bacteria
Basic penicillins
- “beta lactam antibiotics”
- both bacteriostatic & bacteriocidal based on dose
- narrow spectrum to a very few pathogens
Broad spectrum penicillins
- “aminopenicillins”
- bactericidal
- against both gram + and -
indications of broad spectrum penicillins
- LRI
- otitis media
- sinusitis
- UTI
Penicillinase-resistant penicillin
- “antistaphlococcal penicillin”
- bactericidal
- against gram + only
extended spectrum penicillins
- “antipseudomonal penicillins”
- bactericidal
- against gram - only
indications of extended spectrum penicillins
infections of
- bone
- skin
- respiratory
- urinary
beta-lactamase inhibitors
combine with broad spectrum antibiotics to extend the antimicrobial effect
indications for beta-lactamase inhibitors
penicillinase producing staph
labs with amoxicillin
incresed AST, ALT, BUN/Cr
food interactions with amoxicillin
decreased effect with acidic fruits and juices
side effects of amoxicillin
n/v/d, rash, edema
adverse: blood dyscrasias, bone marrow depression, hemolytic anemia, respiratory distress
indication of augmentin
- sinusitis
- pneumonia
- bronchitis
indication of zosyn
(reduce the dose in those with renal insufficiency) infections of -bone -joint -stomach -skin -UTI -pneumonia
drug interactions with penicillins
- amoxicillin&icillin decrease the effectiveness of oral contraceptives
- K+ supplements increase K+ levels when taken with Potassium penicillin G or V
- penicillin and aminoglycosides are inactivated when mixed together in IV fluids
side effects of penicillins
n/v/d, rash, itching, fever, chills adverse: -superinfection (notify MD - usually when broad spectrum drugs are taken 7+days) -hypersensitivity -laryngeal edema -wheezing -hypotension
nursing interventions for penicillins
- C&S before med administration
- monitor closely for bleeding
- observe for allergic reaction - usually 2nd dose
- increase fluid intake
- assess for superinfection (stomatitis & vaginitis)
cephalosporins
- beta lactam structure
- bactericidal
- similar to penicillin
indications of cephalosporins
infections of
- respiratory
- urinary
- skin
- bone
- joint
- general infection
side effects of cephalosporins
-n/v/d
-GI distress
-HA
adverse
-nephrotoxicity (in renal failure)
drug interactions with cephalosporins
- alcohol may cause a really bad hangover (flushing, dizineess, HA, n/v, muscular cramps)
- decreased effect of oral contraceptives
- uricosurics will decrease excretion of cephalosporins causing accumulation & toxicity
