prenatal care and screening, pregnancy health, illness during pregnancy, meds or drugs during pregnancy, form of delivery and/or complications, birth weight, estimated gestational age, problems as a neonate
PMH - peds
congenital anomalies, chronic conditions, serious illnesses and/or injuries, hospitalizations, surgeries
immunization status
up-to-date?, parental concerns, serious adverse reactions
vaccine refusal
acknowledge why, educate on risks, benefits, and responsibilities during an outbreak, document - patient education and refusal
SH - peds
where does child live? who lives with them? who cares for them? smokers, pets, and safety
Developmental Hx
developmental milestones, social habits, school progress
developmental surveillance
routine - AAP - 9,18,30 months, autism -18 and 24 months
Peds - diet
breastfeeding/formula, solids, fruits and veggies, table food, beverages and snacks
Meds
OTC, herbal meds or home remedies, prescriptions meds
Problem-focused visit peds
1-2 issues, HPI - I&O, activity, behavior, PMH as relevant, meds and effects, known ill contacts, ask for input
School / Sports physical
PMH - illnesses/injuries, hospitalizations, surgeries, head trauma, FH - sudden death and hyperlipidemia, menarche and LMP
Adolescent interview structure
- adolescent and parent 2. adolescent alone 3. adolescent + parent
Lipid screening
- 9-11 years old, 2. 17-21, 3. as needed or with risk factors
Adolescent psychosocial history
Home, Education/Employment, Eating, Activities, Drugs, Sexuality, Suicide/depresion/self-image, Safety
mature minor doscrine
protects clinicians providing confidential health care to minors in situations not legally addressed, risk must be minimal, tx must be mainline and you must document the adolescent’s capacity to make informed decisions on and emotional and intellectual level
