Pedi ABC’s
Appearance, Breathing, Circulation
A: Is child responsive/unresponsive?
A/B: Does child have compromise or airway/breathing?
C: Does child have pulse or pulse less than 60/min?
CPR?
Inadequate blood volume and/or O2 carrying capacity; “volume problem”. Hemorrhage, dehydration, burns
Hypovolemic shock
Inappropriate distribution of blood volume or flow; “vessel problem”, Sepsis, anaphylaxis, neurogenic
Distributive shock
Impaired cardiac contractility; “pump problem”. Arrythmias, cardiomyopathy, valve disease
Cardiogenic shock
Blood flow to heart is obstructed; “blockage problem”. Tamponade, tension pneumothorax, PE
Obstructive Shock
Altered mental status, altered heart rate (tachycardia), altered temperature, altered perfusion, decreased cap refill, hypotension, bradycardia (late signs)
S/S of Progressing Shock
5 H’s
Hypovolemia Hypoxia Hydrogen ion (Acidosis) Hypoglycemia Hypo/hyperkalemia
5 T’s
Trauma Tension pneumothorax Tamponade Toxins Thrombosis (coronary, pulmonary)
