what is PCOS
A
hyperandrogenism, oligoovulation and polycystic ovaries on US in the absence of other causes

acne, male pattern baldness, hirsutism, acanthosis nigricans, subfertility. insulin resistance and hyperinsulinaemia

what hormones are raised
A
LH in 40%; testosterone in 30%

what other problems can you have
A
diabetes, hypertension

why do the ovaries become large
A
unruptured follicles, endometrium hyperplastic

what risk is increased
A
ovarian and endometrial cancers

diagnosis
A
exlude- thyroid dysfunction, hyperprolactinaemia, congenital adrenal hyperplasia, androgen secreting tumours, Cushings

Q
management
A
smoking avoidance, treat existing problems, metformin, clomifene (induces ovulation), ovarian drilling, COC, hirsutism- anti androgen cyproterone
