what is a hiatal hernia?
occurs when the upper part of stomach pushes up through the diaphragm towards and into the chest region. This causes a backflow of acid.
3 causes of GORD
- delayed gastric emptying
- impaired oesophageal clearance
- hiatal hernia
name 3 sxs of GORD
• A burning sensation in your chest (heartburn), usually after eating, which might be worse at night • Chest pain • Difficulty swallowing (Dysphagia) • Regurgitation of food or sour liquid Sensation of a lump in your throat
what does a manometry measure?
the function of the lower oesophageal sphincter and the muscles of the oesophagus.
what would normal results from a manometry test reveal?
closed sphincter - pressure 15mmHg
open/relaxed sphincter - pressure no less than 10mmHg
what results would a manometry test reveal for a pt with GORD?
weakened closed sphincter pressure of <15mmHg
how is a 24hr pH monitoring test done?
pt has throat catheter in
asked to keep a diary of sxs, when sitting down/up, when they eat
why is a 24hr pH monitoring test done?
to check whether acid reflux is causing the sxs
types of test done for GORD diagnosis
manometry
xray of upper GI - to rule out other causes
endoscopy
oesophageal pH monitoring
what is laparoscopic fundoplication surgery ?
Sowing the top of the stomach and bottom of the oesophagus together to decrease the diameter of the bottom of the oesophagus to prevent acid from refluxing.
