What does it mean to say someone is Alert and Oriented x 4 (A&O x 4)?
They can correctly answer Name, location, day/time, what happened
What are some of the ways we characterize pain?
C)haracter: sharp, dull
O)nset: when did it come on
L)ocation: where is the pain
D)uration: how long has it been going on
E)xacerbation: what makes it worse
R)elief: what makes it better
R)aditation: does it radiate to other parts of the body
What do you include in each of the 4 parts of the SOAP note?
Subjective: patient or bystander information or reports that cannot be validated or measured (ie: Patients age and gender, chief complaint, mechanism of injury, history of illness)
Objective: measurable, diagnostic (ie: physical exam results, vitals, AOx4, Glasgow coma, skin color, pupil responsiveness, and shape, etc)
Assessment: Best efforts diagnostics of what is happening
Plan: What the first responder is going to do for the patient (hike Vs. Helicopter, educated treatment plan)
What are at least five conditions that always need evacuation?
Suspected spine injury
Suspected pelvic injury
Open fracture
Suspected compartment syndrome (extreme pressure in muscle tissue due to internal bleeding)
Hip or knee dislocation
Vascular compromise to extremity
Laceration with tendon or nerve injury
Uncertainty of severity of injury
