Q

Undescended testes management

A
  • Bilateral at birth -> urgent referral peds + surgery within age 6 months if still undescended by 3 months.
  • Unilateral -> review 6-8wks of age, remains undescended at 3 months + surgery.
  • testis descended but retractile = annual follow up in childhood = risk of ascending testes.

Endoscopiv urethrotomy

17
Q

Urethral strictures - features + ix

A

features:
Decreased urinary stream
incomplete bladder emptying
less common symptoms including spraying of urinary stream and dysuria

Ix
uroflowmetry
ultrasound postvoid residual (PVR) measurement

18
Q

Definition of urethral stricture

A

Scar tissue around the section of the urethra causes Narrowing.

19
Q

Def of condyloma accuminatum and what causes it.

A

A benign sexually transmitted disease in wart form.

Caused by:
- HPV type 6

20
Q

WHat can be seen on histology of condyloma accuminatum

A
  • A treelike pattern of fibrovascular core and papillomatous pattern of growth.
21
Q

Risk factors for condyloma accuminatum

A
  • drug induced immunosuppression
    *HIV
22
Q

How does HPV cause problems in condyloma acuminatum

A

Mucotanous surface of the genital

Infects keratinocytes in skin + epithelial cells in muscosa to proliferate. - visible lesion

Causes - koilocytosis hyperkeratosis, parakeratosis, and acanthosis.

23
Q

Treatment for Condyloma acmuniatum

A

Pdophyllotoxin
imiquimod cream
cryotherapy
Electrosurgeyr

24
Q

Def of hypospadias and epispadias

A

Hypo: abnormal urethral opening on DISTAL VENTRAL surface of penis

Epi: DORSAL surface of the penis

25
Q

what is the clinical significance of hypospadias

A
  1. urinary tract obstruction
  2. increased risk of ascending utis
  3. cant normally ejaculate + failure of insemination = sterility.
26
Q

what is hypospadias associated (conditions) with + tx?

A

Undescended testes
malformation of urinary tract
Tx:
corrective surgery - do not circumcise before it

27
Q

A
  1. accumlation of smegma + lack of hygiene (2ndary phimosis)
  2. Balanoposthitis
  3. Balanitis xerotica obliterans
28
Q

treatment : phimosis

A

Circumcision (definitive)
topical steroid creams
good hygiene

In children less than 2 - will resolve w time.

29
Q

Paraphimosis def + causes:

A

def : foreskin retracted behind glans penis BUT cannot come back over glans.

Cause:
Iatrogenic - penis exam, catheters.

30
Q

Paraphimosis : treatment + complications

A

Tx: Circumcision

complications:
- Severe pain
- vasucalr engorgement
- Oedema

31
Q

Balanitis xerotica obliterans - penile lichen sclerosus ; what is it

A

A chronic atrophic mucocutanous condition of epidermis + dermal connective tissue of genital + perianl skin

33
Q

Treatment for BXO

A
  1. circumcison
  2. laser therapy
  3. Topical steroids
  4. Antifungals.
