aztreonam mono if susceptible

meropenem/vaborbactam
imipenem/cilastatin/relebactam
P. aeruginosa
Monotherapy if not in septic shock or high risk of death
Combination therapy (2 agents) if in septic shock or high death risk
Ceftolozane/tazobactam, cetazidime/avibactam,
imipenem/cilastatin/relebactam, cefiderocol if MDR
If patient on valproic acid…
DO NOT USE WITH CARBAPENEM
Acinetobacter
carbapenem or ampicillin/sulbactam if susceptible
Cefiderocol if resistant to carbapenem or ampicillin/sulbactam
Polymyxin (Colistin, polymyxin B)
IV plus inhaled colistin if MDR to all other options
