part 6 Flashcards
What is a manifestation of CAD?
Angina or chest pain as a result of myocardial ischemia.
What is angina caused by?
decreased supply or increased demand of O2
Why does pain radiate down the left arm during angina?
lactic acid is released on the nerve fiber causing the pain to radiate
What are the 2 most common types of angina?
- stable
- unstable
angina usually intermittent with a consistent: pattern onset duration intensity
chronic stable angina
How might the pain and discomfort be described with chronic stable angina?
pressure, heavy, squeezing, choking, suffocating, substernal or epigastric discomfort, jaw, shoulder, down the arms
-not sharp or stabbing
- pain is short lived (3-15 min) and subsides by relieving the precipitating factor
- pain doesn’t change with position or breathing
- pain at rest is unusual
- possible transient ST segment depression on ECG
- controlled by meds
- take meds when angina is anticipated or at 1st symptom
chronic stable angina
What is the chronic stable angina pain assessment?
PQRST P- precipitating events Q- quality of pain R- radiation of pain S- severity T- timing
Besides stable and unstable angina what are the 4 additional types of angina?
- silent ischemia
- nocturnal angina
- angina decubitus
- prinzmental’s angina (variant angina)
no symptoms, may be DM related (due to neuropathy)
silent ischemia
at night, may wake in a panic
nocturnal angina
occurs when laying down, relieved by standing or sitting
angina decubitus
-spasm of coronary artery related to increased calcium
Prinzmental’s angina (variant angina)
How do you treat Prinzmental’s angina?
Ca channel blocker or nitrates
