Non Depolarizing Neuromuscular Paralytic
Indications for Pancuronium
Induction or maintenance of paralysis after intubation to assist ventilations. Rapid Sequence Intubation (RSI)
Mechanism of Action of Pancuronium
Binds to the receptor for acetylcholine at the neuromuscular junction, producing skeletal muscle paralysis without fasiculations
Contraindications for Pancuronium
Hypersensitivity, inability to control airway and/or support ventilations with oxygen and positive pressure, neuromuscular disease (myasthenia gravis), hepatic or renal failure
Adverse effects of Pancuronium
Weakness, prolonged neuromuscular block, bronchospasm, apnea, respiratory failure, tachydysrhythmias, transient hypotension, hypertension, PVCs, salivation
Precautions of Pancuronium
Tachycardia will be enhanced when used. With positive chronotropic drugs
Adult dose of Pancuronium
0.06 to 0.1 mg/kg slow IV. Repeat every 30 – 60 minutes as needed
Pedi dose of Pancuronium
0.04 to 0.1 mg/kg slow IV/IO
Duration of action and onset
Onset – 30 seconds with peak effect of paralysis in 3 to 5 minutes
Duration of 45 to 60 minutes.
