16
Q

Why does the Serum lipase and amylase go up?

A

These are the digestive enzymes they are trying to go to the small intestines but they can’t get there because there’s an occlusion so they sit in the pancreas so long that the blood absorbed them.

17
Q

What is the Lipase is most specific in diagnosing pancreatitis?

A

Lipase is the one that is most specific in diagnosing pancreatitis

18
Q

What is the tx

A
  1. ) NPO_, NGT to suction, bed rest
  2. ) pain meds
  3. ) steroids
  4. )Anticholinergics
  5. )antacid
  6. ) proton pump inhibitor
  7. )h2 blockers
  8. )Maintain fluid and electrolyte balance
    9) Maintain nutritional status → ease into a diet
    10) Insulin
    11) Daily weights
  9. )Eliminate alcohol
    13) Refer to AA if this is the cause of pancreatitis
19
Q

Why is the client KEEP them NPO_, NGT to suction?

A

Keep stomach empty and dry. This is very important for these people because if anything gets in their stomach they going to think they need to make more enzymes and this is what makes them hurt

So this will help decrease gastric secretions

20
Q

morphine sulfate

A

narcotics

21
Q

hydromorphone

A

narcotics

22
Q

fentanyl patches(

A

narcotics

23
Q

Why do we give steroids?

A

used to decrease inflammation

24
Q

Why do we give Anticholinergics meds?

A

They dry you up and we want the stomach empty and dry .

25
Q

benztropine

A

(Cogentin®)

26
Q

diphenoxylate/atropine

A

(Lonox®)

Anticholinergics drugs

27
Q

What do you give to decrease stomach acid?

A
  1. ) proton pump inhibitors
  2. ) H2 blockers
  3. ) Antacid
28
Q

How do you maintain nutrition status?

A

Ease into a diet

29
Q

What do we give insulin ?

A
  1. because the The pancreas is sick so it may not be producing adequate insulin 2. They may be on steroids and steroids make your blood sugar go up
  2. The client may be getting TPN
30
Q

Alt and ast are

A

Liver enzyme and they never go down there either normal or they go

31
Q

If the pt, Pitt is long what does it mean

If they are short what does it mean ?

A

Long = bleeding

Short = clotting

32
Q

If Hemoglobin & Hematocrit is up what does it mean ?

If they are down what does it mean ?

A

Up = dehydration

Down = bleeding
