A

increased cholesterol output and decreased bile acid synthesis lead to cholesterol supersaturation and crystallization. Hypomotility and excess mucin propagate crystallization

A

Most due to gallstone obstruction of neck/cystic duct. Also trauma, surgery, burns, postpartum
