Where does the teres major originate and insert?
Origin: Lower, lateral border of the scapula
Insertion: Medial lip of bicipital groove
(innervated by lower subscap nerve)
Where does the teres minor originate and insert?
Origin: Lateral border of scapula
Insertion: Greater tubercle of humerus
(innervated by axillary nerve)
Where does the trapezius originate and insert?
UPPER
Origin: Occiput and nuchal ligament
Insertion: Posterior lateral 1/3 of scapular spine
MIDDLE
Origin: Spinous process of T1-T4
Insertion: Medial border of acromion
LOWER
Origin: Spinous process of T5-T12
insertion: Inferior middle scapular spine
(innervated by spinal accessory nerve)

Origin: Thoracolumbar fascia, iliac crest
Insertion: medial intertubercular sulcus
(innervated by thoracodorsal nerve)
Where does the deltoid originate and insert?
Origin: Lateral 1/3 of cavicle (anterior), Acromion (middle), inferior/posterior scapular spine (posterior)
Insertion: Deltoid tuberosity
(innervated by axillary nerve)

Origin: Lateral ribs 1-8
Insertion: Medial border of scapula
(innervated by long thoracic nerve)

Origin: Transverse processes of C1-C4
Insert: Superior angle of scap

Origin: Spinous processes of C7-T1
Insertion: Root of scapular spine
