Where does the teres major originate and insert?
Origin: Lower, lateral border of the scapula
Insertion: Medial lip of bicipital groove
(innervated by lower subscap nerve)
Where does the teres minor originate and insert?
Origin: Lateral border of scapula
Insertion: Greater tubercle of humerus
(innervated by axillary nerve)
Where does the trapezius originate and insert?
UPPER
Origin: Occiput and nuchal ligament
Insertion: Posterior lateral 1/3 of scapular spine
MIDDLE
Origin: Spinous process of T1-T4
Insertion: Medial border of acromion
LOWER
Origin: Spinous process of T5-T12
insertion: Inferior middle scapular spine
(innervated by spinal accessory nerve)

Origin: Thoracolumbar fascia, iliac crest
Insertion: medial intertubercular sulcus

Insertion: Deltoid tuberosity
