Pain Physiology and Analgesics Flashcards

1
Q

During stress, reactions to pain can be suppressed in favor of more adaptive behavior, this response is believed to be mediated by the release of what?

A

Beta-endorphins–“runner’s high”

Q

Non-Opioid Analgestics

A

Acetaminophen and NSAID’s –These drugs are used for pain of the MS system not effective against deep visceral pain

Q

Opioid/opiate Analgescis

A

Relieve moderate to severe pain, that is more or less constant in duration. (Commonly used after surgery, trauma, MI, chronic pain like cancer)

Less effective in treating sharp intermittent pain, although higher doses will relieve this pain.

Q

Acetaminophen (Tylenol, Datril, Excerdin)

A

Effective against mild to moderate pain (analgesic) and in reducing fever (antipyretic) but is not an anti-inflammatory and doesn’t reduce platelet activity

Q

Non-narcotic Analgesic Nonsteroidal Anti-Inflammatory=Aspirin…what 4 things does it do?

A

Analgesic, antipyretic, anti-inflammatory and has an antiplatelelt effect producing a blood thinning action (can be used for MI and TIA)

Q

Non-aspririn NSAID’s drug include:

A

Ibuprofen (motrin/advil/nuprin)

Q

New NSAIDs are continuing to be devleoped to work on what receptor?

A

Want to effect COX-2 over COX-1 (bad GI stuff) Sidto reduce the magnitude of the side effects. (ex: Celebrex)

Q

Opioid/Opiate analgesic inhibit what?

A

Inhibit synaptic transmission in the key pain pathways of the SC and brain. Peripherally they decrease excitability of primary afferent neurons.

Q

Examples of Strong Opioid Agonsit

A

Fentanyl (K-fed probably takes strong opioids)
Dilaudid
Demoral
Methadone (Dolophine)

17
Q

Examples of Mild to Moderate Opioid Agonist

A

Codeine
Hydrocodone
Oxycodone
Antagonist: Narcan

18
Q

Side Effects of Opioids

A

Constipation, euphoria, mental slowness/drowsiness, significant resp depression, orthostatic hypotension

19
Q

Most optimal deliver for pain meds

A

Epidural or intrathecal delivery

20
Q

Abstinence Syndrome

A

Occurs due to withdrawl from opioids, includes body aches, diarrhea, fever, irritability, N/V, runny nose, shivering, sweating, tachycarida
