O2 + CORD = Potential Hypercarbia mechanisms =

A

reversal of hypoxic pulmonary vasconstriction, high levels of CO2 in poorly ventilated alveoli to diffuse back into circulation.
Decreased ventilatory drive
Decreased CO2, buffering capacity of HB
Absorption of CO2 from alveoli beyond obstructed airways.
The higher desity of O2 compaired with air causing increased work of breathing.
Signs of Increased CO2 = confusion, drowiness, agitation, < LOC

Oxygen administration + Bleomycin tx or Paradquat poioning

A

only give to those <88% - Bleomycin is a chemotherapy drug and become sensitive to Oxygen.
Animal experiences = makes worse however in Humans -to give to ensure SPO2 : 94-97 %

Cyanosis =

A

cyanosis is blue discolouration of skin or mucus membranes. Haemoglobin not having oxygen bound to it.
Cyanosis = < 80%

High Risk with Oxygen =

A

People who are CORD, Morbid Obesity, home O2, Neonates, Home CPAP or BIPAP
High risk also includes pt’s that may have Carbon Dioxide clearance

Oxygen Flow Rates =

A
NP: 1-4 Lper Min
Simple Mask: 6-8 L per Min
Neb: 8 L per Min
Reservior Mask: 10-15 L per min
Manual Ven bag: 10-15 L per Min

A

Only administer O2 in confined space if:

  1. clinical indication is very strong
  2. fire service present to monitor oxygen concentration keep under 24%.

Paraquat - use to keep O2 94-97%.
