What are some common conditions of the pelvis?
Pelvic fractures = most common
Neoplasia/osteomyelitis
Fractures are the most common skeletal problem in the pelvis. What causes this?
Trauma - RTA
Assess for other injuries first - only treat when patient stabilised
When suspecting a pelvic fracture after an RTA, what should also be examined/assessed?
Urinary tract - is patient urinating?
Neuro exam
Reflexes
How can pelvic fractures be treated?
Conservative - 75% recover (small animals > large dogs)
Surgery
Pelvic fractures requiring surgery should be operated on within what time frame?
5 days
Which pelvic fractures are suitable for conservative treatment?
Non-weight bearing parts:
Pubis, ischium, wing of ilium
What conservative management should be advised for pelvic fractures?
Cage rest for 4-6 weeks
If non-ambulatory, frequent turning and soft bedding
Check bladder 4X day if not urinating consciously
Analgesia - opioids, NSAIDs
Which pelvic fractures are suitable for surgical treatment?
Weight bearing areas:
Acetabulum
Iliac shaft
Sacroiliac joint
Ipsilateral fractures of ilium, pubis, ischium
(Or fractures causing marked stenosis of pelvic canal)
Sacroiliac separations are commonly seen in which species? After what trauma?
Cats
After RTA
Are sacroiliac separations usually unilateral or bilateral?
Unilateral more common
Are sacroiliac separations painful?
Very - especially if nerve root entrapment
What is the treatment for sacroiliac separations?
Surgery - if painful, non-ambulatory, marked displacement or other contralateral fractures
What surgery can be used to treat sacroiliac separations?
Lag screw fixation
Or transilial pin
How are ilial shaft fractures and acetabular fractures treated?
Surgery - internal fixation (plating)
