Overweight woman recently taking isotretenoin who presents with pulsatile headache, whooshing sound in her ears, and visual changes with papilledema on exam. LP shows increased ICP; disease and treatment?
A
Idiopathic intracranial HTN (IIH): overweight female of childbearing age, recent tetracyclines or accutane. Sxs are HA, vision loss, tinnitus. PE reveals peripheral field defects, papilledema, and CN VI palsy. LP shows increased ICP >250. Stop meds, wt. loss, give acetazolamide.
