A

Cerebral Toxoplasmosis

EBV DNA in CSF = lymphoma; often described as ‘weakly ring-enhancing’ mass that is solitary and periventricular.

Immunocompetent patient has a few weeks of headaches, focal neurological deficits, and a solitary ring-enhancing lesion on brain CT with a fluid collection in the ethmoid sinus following a sinus infection. Most likely cause?

A

Viridian’s strep brain abscess.

Abscess:
50% are direct spread from HEENT infection.
25% are from distal spread (endocarditis/lung).
