pernicious anaemia
Pernicious anaemia is the most common cause of vitamin B12 deficiency. Intrinsic factor is normally produced in the stomach and binds B12 in the gut. Autoimmune destruction of the stomach lining leads to reduced intrinsic factor production impairing B12 absorption. This can eventually precipitate a vitamin B12 deficiency and macrocytic anaemia as is the case here. B12 deficiency can also damage spinal cord myelination resulting in neurological symptoms.
