what is osteochondritis dissecans (OCD)?
pathological process affecting the subchondral bone (most often in the knee joint)
who typically suffers from osteochondritis dissecans?
children and young adults
what is the presentation of osteochondritis dissecans
- knee pain and swelling (typically after exercise)
- knee locking, catching and/or giving way
- ‘clunking’ when flexing or extending knee
what anatomical structure is involved in osteochondritis dissecans if the patient experiences ‘clunking’ on flexing or extending their knee?
lateral femoral condyle
what are the signs of osteochondritis dissecans?
- joint effusion
- tenderness on palpation of the articular cartilage of the medial femoral condyle, when the knee is flexed
- wilson’s sign
what is wilson’s sign indicate?
medial condyle lesion
what is wilson’s sign?
- with the knee at 90 degrees flexion and tibia internally rotated, the gradual extension of the joint leads to pain at about 30 degrees
- external rotation of the tibia at this point relieves the pain
what investigations are done in osteochondritis dissecans?
- x-ray (AP, lateral and tunnel views)
- MRI
what can be seen on x-ray in osteochondritis dissecans?
- subchondral crescent sign
- loose bodies
