what are the features of hip fracture?
- pain
- shortened and externally rotated leg
- non-displaced or incomplete = weight bear
where is an intracapsular fracture?
from the edge of the femoral head to the insertion of the capsule of the hip joint
where is an extracapsular fracture?
- trochanteric
- subtrochanteric
what is the dividing line between the trochanteric and subtrochanteric
lesser trochanter
what system is used to classify hip fractures?
GARDEN system
‘old ladies fall in the GARDEN and break their hips’

what is type I?
- incomplete fracture
- undisplaced

what is type II?
- complete fracture
- undisplaced

what is type III?
- complete fracture
- incompletely displaced (still has boney contact)

- completely displaced
what hip fractures are most commonly associated with blood supply disruption?
stage III and IV
what is the management of an undisplaced intracapsular hip fracture?
- fit = internal fixation
- unfit = hemiarthroplasty
what is the management of a displaced intracapsular hip fracture?
replacement arthroplasty (total hip replacement or hemiarthroplasty)
when is total hip replacement favoured to hemiarthroplasty?
patients are:
* able to walk independently out of doors with no more than the use of a stick AND
* are not cognitive impaired
* are medically fit for anaesthesia and the procedure
what is the management of stable intertrochanteric factures?
dynamic hip screw
what is the management of a reverse oblique, transverse or subtrochanteric fracture?
intramedullar device
