Introduce yourself, ensure privacy and that patient in comfortable, hand hygiene, explain procedure.
What does COLDSPA stand for?
Character, onset, location, duration, severity, pain, and associated factors.
What would we ask to do with associated factors?
Other signs and symptoms, allergies, medical/family/surgical history, current medications, smoking and/or alcohol use, and ADLs.
Before beginning any assessment what do we need to do.
Explain procedure including reasoning for taking notes, gain consent, hand hygiene, and prepare equipment.
Why is it important to take notes when preforming a medical assessment?
So that we can record all the relevant information allowing us to make the correct medical decisions.

Decide which health assessment to preform and provide the rationale for doing so.

Whats a good order to do vital signs in?
Temperature, radial pulse, respirations, pulse oximetry, blood pressure.
What are the four things you need to check you have done when doing documentation?
Include time, date, initials, and early warning score.

The phase of the heartbeat when the heart muscle relaxes and allows the chambers to fill with blood.
Define systolic.
The phase of the heartbeat when the heart muscle contracts and pumps blood from the chambers into the arteries.
