What is OSAS?
Recurrent episodes of complete upper airway obstruction during sleep causing intermittent hypoxia and sleep fragmentation
What are Sx of OSAS?
Daytime somnolence, snoring, poor sleep quality, low libido, morning headache
What are risk factors for OSAS?
Obesity
Retrognathia or micrognathia
Wide neck circumference
What Ix in OSAS?
Polysomnography: full or limited. Involves measuring sats, EEG, respiratory airflow, snoring, thoraco-abdominal movement
TOSCA
Epworth sleepiness score
Apnoea-hypopnoea index. > 15 is diagnostic, 5-15 is diagnostic with symptoms
What is Rx of OSAS?
Weight loss, avoid alcohol
tonsillectomy can help
CPAP to keep airways open
Mandibular advancement device if CPAP not tolerated
Surgery for face if severe
MUST inform DVLA if daytime sleepiness
What are complications of OSAS
Hypertension R heart strain CVD Stroke RTA Type II respiratory failure
