purpose of 1st ray cutout
-encourages plantarflexion of 1st ray to increase dorsiflexion at 1st MPJ to allow for resupination
1st ray cutout cons
-lose some control b/c lose the distal medial corner of the device which is a control point
does a 1st ray cutout with 2-5 bar posts correct frontal plane corrections
NO!
-lifts up the MT in order to allow the 1st ray to function properly
1st ray cutout with 2-5 bar posts are for patients who
function in a PF position/significantly PF 1st ray
would you use a 1st ray cutout with 2-5 bar post for someone with hallux rigidus?
NO!
-pt would collapse over the medial side of the device and be in a lot of pain
purpose of Kinetic (Dynamic) wedge
- takes advantage of when the 1st ray PF it also everts
- triangular wedge encourages 1st ray to PF and evert to allow for better 1st ray function
purpose of Dancer’s pad
- used to accommodate 1st MT head issues
- used for hyperkeratosis of 1st MT head or for sesamoditis
can dancer’s pad be used for biomechanical dysfunctions?
NO, use to ACCOMMODATE 1st MT head only
what is Morton’s extension
-a 1st ray modification that extends to the IPJ of hallux and pushes up on the 1st MT head
what is the purpose of Morton’s extension
-prevent hallux dorsiflexion
when do you use Morton’s extension
-when you want to limit MT head dorsiflexion
types of plantar fasciitis modifications
- plantar fascial groove
- heel lift
- heel spur accommodation
what is a plantar fascial groove
- mark prominent plantar fascia on foot so it comes out on the negative which is transferred to the positive
- grooves are addressed on the device so the pt can tolerate their orthoses
purpose of heel lift
-reduce the compensation for equinus
purpose of heel spur accommodation
-acts similar to a heel lift to reduce tension off of the plantar fasciitis
if a heel spur accommodation works for other reasons other than a heel left you should
- rethink your diagnosis
- maybe there is a bursitis
