- Ensure pt details match details supplied for appliance
- Check appliance matches design specifications
- Inspect appliance & run your finger over all surfaces looking for sharp/potentially traumatic areas
- Check integrity of wirework
- Insert appliance into pt’s mouth, immediately looking for areas of blanching/soft tissue trauma
- Check posterior retention (Adam’s clasp). Firstly flyovers, then arrowheads are correctly engaging appropriate undercuts.
- Apply same principle to anterior retention
- Activate appliance
- Demonstrate to pt correct procedure for insertion/removal of appliance (ensure that pt demonstrates this correctly)
- Book a review appointment 4-6 weeks later
what information should you give to a patient receiving a URA?
bulky
saliva
speech
sore
wear 24/7
clean after meal times
remove for sport
no hard/sticky food
careful with hot food
missed appointments/non-compliance
emergency contact
- appliance will feel big and bulky- this is normal and they will get used to it
- It may cause initial excessive salivation- will pass within 24 hours
- May impinge speech for a short period of time- practise reading a book aloud
- May cause initial discomfort or ache-this is normal and means appliance is working
- To be worn 24/7 including meal times/sleep
- Remove after every meal and clean with a soft brush
- Remove and store in a protective container when participating in contact or active sports
- Avoid hard or sticky foods that may damage the appliance and be cautious with hot food or drinks
- Missing appointments and non-compliance will significantly lengthen treatment time
- Provide emergency contact details in case any problems arise

describe the checklist for an Adam’s clasp
- Arrowheads must engage mesial and distal undercuts except when tooth is rotated
- bridge of clasp stands clear of tooth at approximately 45 degrees to crown
- Arrowheads are parallel
- Arrowheads should be 45 degrees to tooth surface
- Arrowheads must not touch adjoining teeth
- Bridge should not protrude above occlusal surface
- wire (flyover) fits closely over contact areas N.B. if there is no adjacent tooth present, flyover must cross above contact area
- There should be clearance of 0.5mm-1.0mm between wire and tissue in palate
- There must be tags present at ends of wire to supply addition mechanical retention within baseplate
- gingival margin of any models should only be trimmed if tooth present has not fully erupted
