What is osteomyelitis?
inflammation of the bone and medullary cavity usually localised in long bones
What is the most common cause of osteomyelitis?
Staph
Mainly staph A
Gold standard test for osteomyelitis?
bone biopsy
Management of osteomyelitis?
best to wait on micro results, DO NOT TREAT empirically unless the patient is systemically unwell
need surgery to remove infected tissue, drain and debride as no antibiotic will work on the necrotic centre
6 weeks antibiotics tailored to organism
Main cause of septic arthritis?
staph A
Symptoms of osteomyelitis?
red, hot, swollen, loss of function at site, may have fever
Symptoms of septic arthritis?
joint is red, hot, swollen and agonizingly painful, it is immobile
Investigation of septic arthrits?
aspirate joint and send fluid to micro (important to check for crystal to rule out crystal arthropathy)
blood cultures
Management of septic arthritis?
if staph A - IV flucloxacillin
might need to wash out the joint
