Orlistat inclusion criteria
- Should only be sold to overweight adults with a BMI >28kg/m2
- Max daily dose of 180mg in divided doses
- Treatment with OTC orlistat should not exceed 6 months
MOA
a lipase inhibitor, reduces the absorption of dietary fat.
Orlistat Indication
- mildly hypocaloric diet in
- (BMI) of 30 kg/m2 or more
- BMI of 28 kg/m2 or more in the presence of other risk factors such as type 2 diabetes, hypertension, or hypercholesterolaemia
Dose
120 mg up to TDS
Administration
immediately before, during, or up to 1 hour after each main meal,
Missed dose
if a meal is missed or contains no fat, the dose of orlistat should be omitted
When to continue treatment
continue treatment beyond 12 weeks:
- only if weight loss since start of treatment exceeds 5% (target for initial weight loss may be lower in patients with type 2 diabetes),
What are the ADRs of orlistat use
Abdominal pain
flatulence
faecal incontinence
fatty stools
Orlistat and diet
- May potentially impair the absorption of fat soluble vitamins
- Should be taken in conjunction with low fat, mildly hypocaloric diet
What is very rarely associated with orlistat use?→
hepatitis -
abdominal pain
dark urine
jaundice
itching
Contraindications
Cholestasis; chronic malabsorption syndrome
Breast feeding
Avoid
Pregnancy
