opportunistic pathogens Flashcards
definition
refers to those fungi that normally would not cause infections in otherwise healthy people but are able to cause infection under certain circumstances such as immunodefficiency, cancer, organ transplant, neutropenic patients, diabetes, debilitated patients and patients on long term antibiotics.
4 main types
Candida sp, Aspergillus sp, Cryptococcus sp, Pneumocystis jirovecii
risk factors
malnutrition, recurrent infection, chemotherapy, immunosupressing agents, organ transplant , skin damage, antibiotics, pregnancy, neutropenia
key infections caused by opportunistic pathogens
cryptococcosis, Aspergilliosis, Pneumocystis, candida albicans
an emerging opportunistic apthogen
candida auris- first reported in japan in 2009, causes both superficial and systemic disease. key concerns of drug resistance and hospital outbreaks.
cryptococcosis causative agents
cryptococcus neoformans and gatti
Cryptococcus neoformans
encapsulated yeast and obligate aerobe
Cryptococcus gatti
dimorphic, capsulate yeast, found in soil
cryptococcosis route of infection
inhalation of spores and sexual style produces more spores
cryptococcosis causes
pulmonary cryptococcocus- often asymptomatic .
-> an intracellular pathogen that persists
cryptococcosis and AIDS
5-10% in USA, has a high mortality rate of 30%
symptoms of cryptococcosis
fever, non productive cough, chest pain
pneumocystis causative agent
Pneumocystis jirovecii
disease cause by pneumocystis
Pneumocystis pneumonia- dry cought
who’s most likely to get Pneumocystis
immunocompromised patient and AIDs defining disease
mortality in Pneumocystis
5-40%
pneumocystis route of infection
inhalation
pneumocystis was originally miss classed as a
protozoan- however is a non-culturable in vitro obligate parasite
why is pneumocystis genome difficult to sequence
laks genes for amino acid biosynthesis–> usually scavenges from the host
aspergillosis causative agent
A.fumigatus, A.flavusmost Aspergillus species do not cause disease
aspergillosis route of infection
inhalation (normally inhale 100-200 spores / day without disease)
types of disease associated with aspergillosis
allergy (asthma), toxins, invasive bronchopulmonary aspergillosis
invasive bronchopulmonary aspergillosis usually occurs in
immunocompromised people
disseminated aspergilliosis (immunosuppressed patients) has a mortality rate of
60-90%
