what is codeine used for?
mild to moderate pain when paracetamol is not longer effective
can also be used be anaesthatists post operation for analgesia

codeine and dihydrocodeine work as they are metabolised in the liver into small amounts of morphine and dihydromorphine. agonists of opoid receptors
but what can occur in 10% of people?
10% of Caucasians have the cytochrome p450 2d6 variant where codeine has no effect. therefore a synthetic analogue tramadol can be used.

nausea constipation drowsiness and dizziness
overdose-neurological and resp depression
CI
hepatic and renal impairment lower dose
significant resp disease
lowers seizure threshold should avoid in epileptics->

sedating medications such as Tricylic antidepressants,
benzodiazepenes, anti-psychotics
tramadol- TCA, SDRI
one thing to be aware of
never give IV
can provide a severe reaction like an anaphylactic like reaction but is not allergic based.
