non-steroidal anti-inflammatory drugs
indication for use
mild to moderate pain as an alternative for paracetamol
pain caused by inflammation such as in gout, arthritis
MOA
inhibits prostaglandin synthesis from arachidonic acid
by inhibiting the COX enzyme (cyclooxygenase )
whats the difference between cox1 and cox2
cox1 is needed for renal perfusion, preserving integrity of gastric mucosa, inhibiting thrombus formation
cox2 is the inducible form which is present in inflammation and triggers prostaglandin synthesis and causes inflammation andpain
name some NSAID include a cox2 specific NSAID
naproxen and ibruprofen
cox2 specific etoricoxib - benefit is that it reducesside effects
AE
GI toxicity
renal impairment
hyperensitity reaction -bronchospasm
angioedema and worsen HF and hypertension
DI
increased risk of bleeding with warfarin, aspirin and anticoagulants, SSRI
CI
renal and hepatic impairment known hypersensivitiy GI bleeds/PEPTIC ULCER DISEASE heart failure GI ulceration with corticosdeteriods renal impairment with ACEi and diuretics
who should be given what?
