Name the three FDA approved indications for Zofran/Zuplenz.
- For the prevention of nausea and vomiting associated with initial and repeat courses of moderately emetogenic cancer chemotherapy.
- For the prevention of nausea and vomiting associated with radiotherapy in patients receiving either total body irradiation, single high-dose fraction to the abdomen, or daily fractions to the abdomen.
- For the prevention of postoperative nausea and/or vomiting in patients where nausea and/or vomiting must be avoided
What is the FDA approved dose for patients using Zofran for nausea/vomiting associated with cancer chemotherapy?
In adults, give 8mg twice daily beginning 30 minutes before the start of chemo with the second dose coming 8 hours after the first. Continue twice daily dosing for 1-2 days after completion of chemo. In children 4-11, use 4mg dose given TID.
What is the FDA approved dosing for patients using Zofran for N/V associated with radiation?
8mg given TID. Begin 1-2 hours prior to radiotherapy and continue for 1-2 days after completion
What is the FDA approved dosing for Zofran when used for post-operative N/V?
16mg given 1 hour prior to anesthesia
In patients with hepatic impairment, what is the maximum daily dose of Zofran?
8mg daily
What are the adverse effects of Zofran?
Headache, fatigue, constipation
What should the RPh tell the patient when consulting on Ondansetron HCl?
Do not remove chew tabs from wrapper until just prior to use, DO NOT push them through. Store in a cool, dry place away from sunlight & children. If a dose is missed, take ASAP.
What are the possible drug interactions for ondansetron HCl?
use with apomorphine has caused severe hypotension and loss of consciousness. The risk of serotonin syndrome is increased when used with other drugs that affect serotonin receptors.
What are the contraindications for ondansetron HCl?
Contraindicated with apomorphine. Avoid use in patients with known congenital long QT syndrome. ECG’s monitoring is recommended in patients with electrolyte abnormalities, congestive heart failure, bradyarrthymias, or patients taking other medicinal products that lead to QT prolongation. Serotonin syndrome has been reported with selective 5-HT3 receptor antagonists. Do not use with phenylalinine.
