OMPHALOCELE AND GASTROCHISIS Flashcards
Describe omphalocele
Omphalocele is the presence of gelatinous
Omphalocele Gastrochisis transparent sac covering the eviscerated
Commoner in children C o m m o n in
children abdominal contents
Layers of sac: Peritoneum, Amnio
Eviscerated bowel i s Eviscerated
b o w e l Wharton’s jelly
Associated syndromes: omphalocoele
*macroglossia
* gigantism,
*beckwinhwieldemann
* Caudal syndrome, Pentalogy of cantrel(diaphragmatic defect)
* pericardial defect,
* epigastricomphalocele, ectopic cordis, sternal defect), Trisomy 13, 18, 21
Classification omphalocoele
Based on size
a. omphalocele minor: sac diameter is <5cm. umbilical cord is attached to the midpoint.
Content is just the midgut
b. omphalocele major: sac diameter >5cm. umbilical cord is attached to the pole. Cent is other abdominal viscera
-based on syndrome
a. syndromicomphalocele
b. non syndromicomphalocele
-based on state ofs a c a. intact sac
b. ruptured sac
GASTROCHISIS definition
*Evisceration of bowel and is often located to the right of the umbilicus without any sac covering it
Investigation of gastrochisis
Investigations
Babygram, RBS, Echocardiography, Chest X- ray, Karyotype/Genetic screening, Amniotic fetoprotein (AFP)
Similarities betweenGastrochisis and Omphalocele
Both congenital defects
-both are anterior abdominal walldefect
-affects babies (males)
-extremes of age of mothers are at risk
-temperature, swelling and pain are common features seen in both
Differences between omphalocoele and gastrochisis
Gastrochisis :
*children born to teenage multiparous women mothers
* Eviscerated bowel sac covering it
*it is isolated
Omphalocoele
*eviscerated bowel is covered with a sac (peritoneum ,Wharton jelly sac amniotic fluid
* it is more associated with symptoms
