Croup: Path, Age, Sx
Parainfluenza
3 mos-3 yrs
Viral Prodrome
Barking, seal-like cough, Stridor
Croup Dx:
Xray: Steeple Sign (nonspecific)
Give RAcemic Epi and see if they get better
Croup Tx:
Mild: mist
Moderate: Racemic Steroids, Oxygen
(CB says dexamethasone and racemic epi)
Severe: Moderate and admit.
Bacterial Tracheitis: Path
Age
Sx
Path: Staph Aureus
Pt. Wide, avg 4 yrs.
Croup that does not improve with croup tx (racemix epi)
Bacterial Tracheitis: Dx
Xray- steeple sign (nonspecific)
Racemic epi does not work.
Tracheal culture
Bact Trach: Tx
IV ABX
F/u: Scope
Epiglottis: Path, Age, Sx
H. flu- vaccine 3-7 yo Sick, rapid onset, high spiking fever, tripoding, drooling, accessory muscles Hot-potato-voice
Epiglottitis Dx
x-ray- thumbprint: (nonspecific)
Visualize cherry-red epiglottis
Epiglottitis Tx:
ET tube OR (you touch it, it will blow up)
IV ABX
Retropharyngeal Abscess Path and sxs
Path: oral flora Sick, rapid onset high fever, drooling, neck extended *Ant chain LN *Tender Mass= Abscess
Retropharyngeal Abscess DX and TX
CT scan
TX: Incision and drain/ aspiration
IV ABX
Peritonsillar Abscess
Path: oral Pt: 10 yo or older Sx: hot-potato-voice Drooling, sore throat, *uvula deviation
Dx: Clx
Tx:
DRain and ABX
