URI bugs: Strept, morxilla, H-flu

MIddle ear: tympanic membrane is infected

Unilateral ear pain
Relieved by pulling pinna
Loss of light reflex
Erythematous, fluid behind TM.

Otitis Media Dx and TX

A

DX: pneumatic insuflation

TX: 1st Amoxicillin
Recur: Amoxicillin and Clav
Recur: 3 x/ 6 mos or 4x/ 12 mos- Tympanoplasty
Pen-All: Cefdinir…Azithromycin

Otitis Externa

A
Path: Outer ear
Swimmer-pseudomonas
Digital-Staph
Pt: unilateral ear pain
Worse with pulling pinna
Erythema, Angry canal

Dx: Clx
Tx: Nothing or abx (ciprofloxin for pseudo) and steroids to make them feel better.

Mastoiditis

A

Path: URI bugs, especially with tympoplasty.

They already have acute otitis media, +swelling behind ear, anterior rotated ear.

Dx: Clx… You don’t CT

Tx: Already treating Otitis Media. Surgical.
