Started with RDS
from decrease surfactant
decrease alveoli.
Pt: increase oxygen demands,
increase FiO2 for 28 days or more (transition from RDS to BPD)
Lung protective strategy
Dx: X-ray shows ground glass opacities
Tx: surfactant (post-natal)
Steroids (Ante-natal)
F/U: DPLD (diffuse parenchyma lung disease)
Retinopathy of Premie
Path: neoangiogenesis, increase FiO2
Dx: Eye Exam
Tx: Laser
F/U: Glaucoma
Intraventricular Hemorrhage
Path: Highly Vascular Ventricles. Changes in BP
Pt: Premie asx Inc ICP (Will produce mass effects)
Dx: Cranial Doppler
Tx: Decrease ICP…. VP shunts and drains
F/U: mental retardation, Seizure
Necrotizing entercolitis
Path: dead gut
Pt Premie…. Bloody BM
Dx: X-ray= pneumatosis intestinalis
TX: NPO, IV Abx (against gram negative), TPN
F/U: surgery
