DSM-5 criteria
experiencing obsessions and/compulsions that are time-consuming >1hr a day and cause significant impairment not due to substance, medical condition, or another mental disorder.
what are obsessions and compulsions?
obsessions: repetitive, intrusive, anxiety-inducing thoughts, images, or urges that the patient attempts to neutralize by performing compulsions.
compulsions: repetitive behaviors or mental rituals that the patient feels the need to perform in response to obsessions to lower anxiety

OCD vs OCPD
OCPD are obsessed with details, control and perfectionism
OCD are distressed by their symptoms (ego-dystonic)
OCD Tx?
treatment resistant?
medications and CBT
1st line: SSRI usually in high doses
2nd: SNRI or TCA (clomipramine)
can augment with atypical antipsychotics in severe cases
Tx resistant: neurosurgery (cingulotomy) or ECT
whats CBT focused on in OCD tx?
focuses on exposure and response prevention.
prolonged and graded exposure to the compulsion-inducing stimulus and prevention of the relieving compulsion
