what is a Class A Mishap?
total cost of reportable material property damage is $2 million or more
or injury or occupational illness results in fatality or permanent total disability
what is a Class B Mishap?
total cost or reportable material property damage is $500,000 or more but less than $2 million
or injury or occupational illness that results in permanent PARTIAL disability
or three or more personal are inpatient hospitalized
what is a Class C Mishap?
total cost reportable material or property dame is $50,000 or more but less than $500,000
non-fatal injury causes any loss of time beyond the day/shift on which it occurred
non-fatal illness or disease that cause loss of time from work/disability at any time
reporting purposes mishaps are those which result in 5 or more lost workdays beyond the date of injury or illness
what does PPE stand for?

Why was the use of PPE established?
as a last line of defense in the event of equipment breakdown, failure, misuse to include hazardous environments and working conditions that would expose the worker to a hazard.
