Aphthous ulceration
Small, painful, but resolve quickly - very common
Lipoma
Ball of fat, yellow hue, not painful, squishy.
Mucocele
Balloon of saliva usually as a result of trauma to the salivary gland.
Epulis (“swelling on the gingivae” in this instance a pyogenic granuloma)
Caused by an edge of amalgam restoration.
Leaf fibroma (develops from a small peduncle (stalk) attaching to the anterior part of the hard palate)
Often found with denture wearing patients
Denture induced hyperplasia
hyperplastic extrafolds of tissue that engulf the inner and outer surface of the denture.
Treatment for this case
- Systemic antifungals to try and clear the candida infection
- Improved dental hygiene
- Reduction of the soft tissue
- Replacement of the denture with a soft lining (i.e tissue conditioner)
- Allow a period of healing, then have a new denture made.

Denture-induced
Torus palatinus
Not a problem, but can be at risk of tramua
Tori mandibularis
Not a problem, but can be at risk of tramua

Smoking - causes hyperkeratosis (the opposite of aphthous ulcers

Clinical examination shows the ulcer has reverted margins and there is reduced mobility of her tongue. Which one of the following is the most likely diagnosis?
These scenario is consistent with the presentation of squamous cell carcinoma. Lesions associated with leukoplakia and erythroplakia do not ulcerate until malignancy intervenes. Ulcerative lichen planus is painful and oral submucous fibrosis is associated with a history of betel quid use.
Acanthosis
is a thickening of the epidermis and elongation of the rete ridges due to thickening of the spinous layer +/- enlargement of rete pegs.
