Definition

A

Obstetric liver condition that causes a build up of bike acids and liver dysfunction. Chloeostasis due to high levels of Eostrogen

Physiology

A

Liver cells are unable to hold the enzymes which then leak into the blood stream.
Bile acids play a role in digestion and help break down food. The bile acids leak into the blood and can cause teratogenic effects.

Signs and symptoms

A

Pruritus (nocturnal) Raised bile acids and abnormal LFTS. Right upper quadrant pain
, pale stools, jaundice
UTI

Risk factors

A

Multiple pregnancies
Subsequent pregnancies
Make sure to rule hepatitis, liver ultrasound

Complications

A

Gall stones, recurrent OC, contraceptive chloeostasis, vitamin k deficiancy, pph liver failure

A

Bile acids cause Vaso constriction of the placental veins causing premature delivery and still birth, meconium

Management

A
Weekly LFTS, bile acids
Antihistamines for itching and calmoline lotion 
Vitamin k orally 
Regular USS monitor movements
Risk of pph so active third stage
