What are tocolytics?
Medications that slow down uterine contractions to delay premature labor. Delay for about 48 hrs.
Indomethacin, nifedipine or terbutaline
What is the most important risk facot for postpartum endometritis?
C-section!!
Also
prelabour rupture of membranes >24hrs
active labor >12 hrs
frequent pelvic exams
internal fetal monitoring
What is the tx for endometritis?
Clindamycin + gentamicin
What does it mean for a woman to be Rh negative?
She has an absent D antigen. If she becomes pregnant with a baby that is RH-positive. She will develop anti-D antibodies for following pregnancies not the 1st.
anti-D antibodies attack the RBC of the baby = hemolytic disease.
What is the presentation of hemolytic diease is a newborn 2ndary to RH incompatibility?
Skin edema, ascites, pericardial effusion, pleural effusion & severe anemia hem <5g/dL
When is anti-D immune globulin administered to RH negative pregnant women?
28 weeks gestation and after delivery if the coombs test shows fetal RBC covered w maternal antibodies.
What is pre-eclampsia?
New onset HTN (SBP >140 or DBP>90) & proteinuria (>300 mg per 24 hr urine collection) OR evidence of end-organ dysfunction. after 2o wks in a previously health person.
