Syphilis treatment complication

A

Jarisch-Herxheimer reaction

Fever, nausea, tachycardia, headaches, flushing after first dose of benathine benzylpenecillin for syphillis

Don’t stop the Abx just give paracetamol (IV)

How to differentiate between the causes of genital ulcers

A

Painless ulcer and painless lymphadenopathy= syphillis

Painless ulcer and painful lymphadenopathy= lymphogranuloma venereum

Painful ulcer and painful lymphadenopathy= chancroid/ HSV (chancroid has ragged edge, deep vs HSV is small with red edge)

How does meig’s syndrome present

A

Benign ovarian tumour

Ascites

Pleural effusion

Treat with thoracentesis and paracentesis to drain off fluid and respect the fibroma to correct the UL cause
