A

cannot convert pyruvate into acetyl coa–> can’t make citrate –> can’t continue down TCA–> LESS ATP. ATP DEPLETION. common symptoms are wernickes korsakoff and beri beri

Pt in ER is given 5% dextrose with normal saline; all of a sudden pt develops confusion, nystagmus, ataxia, and opthalmaplegia. What happend?

A

pt had thiamine deficiency which is necessary for metabolism of glucose. Giving IV glucose with a thiamine def can worsen the deficiency and cause Wernicke encephalopathy. give IV thiamine with IV glucose.
