What is involved in the assessment part of the nursing process?
Gathering information (health problems and potential health problems)
Subjective data - info from owners (mood, pain etc.)
Objective data - measurements (HR, breathing, temp, BP)
Understanding normal - to make patient more comfortable, food types, surroundings etc.
What is involved in the planning part of the nursing process?
Analyse info gathered
Strengths (don’t require intervention)
Risk factor, may hold patient back/ prevent recovery
Functioning problems = actual problems
Set goals, to address actual and potential problems
Decide on a treatment
Medications required etc.
What is involved in the implementation part of the nursing process?
Carrying out plan
Reaching goals
What is involved in the evaluation part of the nursing process?
Evaluate goals (have they been met? if not, why?)
Reflect on evaluation
Has health improved?
Patient is then re-assessed
What does SMART stand for?
Specific Measurable Achievable Realistic Time-bound
Why is it important to record all interventions?
As if you don’t, it hasn’t been done and someone else will do it causing a repeat
What is the re-assessment period dependant on?
The patient
More critical = more regular re-assessment
What are some advantages of care bundles?
Standardisation between patients
Evidence of improved patient outcomes when bundles are used
Promotes efficient use of resources by utilising proven and effective instructions
Can potentially limit fringe practice which may be unproven or dangerous
What are some disadvantages of care bundles?
May not take into account indivual patient
May limit indivual clinicaian autonomy or indepence
Results in additional admin burden for staff
Unintended side effect of standardisation may discourage best clinical practice
CAn be unduly influenced by external forces
Indivual elements may have limited evidence
