What is involved in the assessment stage?
Asking owner, VS etc for information
Gathering subjective and objective data about the patient
Obtain a health history and determine the patient’s ‘normals’
What is a nursing diagnosis?
Actual and Potential problems the patient may face
Prioritise the list of problems
Analysis of the collected information
What is involved in the planning stage?
Setting SMART goals which are clearly defined and measurable.
Planning the interventions (ideally start with minimal, review, and then progress to more severe if needed
What is the implement stage?
Carrying out the interventions and recording
Constantly reviewing and monitoring the patient
What is the evaluate stage?
If the goals aren’t being met, why?
Identify how effective the interventions are at meeting the goals.
What comes after the evaluation?
Reassessment and the cycle can start again.
Name a nursing model that you will use to make a care plan

Describe the Ability Model
- Eat adequate amounts?
- Drink adequate amounts?
- Urinate normally?
- Defaecate normally?
- Breathe normally?
- Maintain body temperature?
- Groom itself?
- Mobilise adequately?
- Sleep/rest?
- Express normal behaviour?
Considers Cultural, financial and owner compliance as affecting factors.
Important to consider where the patients sits in their lifespan.
What is a nursing model generally?
A tool to identify patient needs and assist them in carrying out their normal daily activities
